The minor and parent (legal guardian) must both be present and agree on the selected artwork or tattoo design; it&#8217;s placement and size, coloring, etc.<\/p>\n<p>2. The minor and parent (legal guardian) must BOTH be able to present a state issued ID, e.g., drivers license and learners permit.<\/p>\n<p>3. The state issued IDs of the minor and the parent MUST&nbsp;have the <span style=\"text-decoration: underline;\">same<\/span> last names and the <span style=\"text-decoration: underline;\">same<\/span> residential address.
